COVID-19 Declaration Form

Celsius Design is taking a highly precautionary approach to manage the spread of COVID-19, which is in line with the latest national and local health advice. As part of this approach, we are restricting any non-essential visits to the office. If the visit is considered essential, we ask each visitor to confirm the following before they enter:


    Symptoms include but are not limited to; fever, coughing, sore throat, headaches, raised temperature or difficulty breathing.
